 GP Management Plan (GPMP) Referral Process

GP Management Plans (formerly known as Chronic Disease Management Plans [CDMP] or Enhanced Primary Care [EPC]) enables GPs to plan and coordinate the health care of their patients with chronic or terminal medical conditions, including patients with these conditions who require multidisciplinary, team-based care from Allied Health Professionals. The program entitles you to up to five (5) Allied Health sessions subsidised through Medicare per year.

 
 

Whether a patient is eligible for GPMP services is a clinical judgement made by the GP so we encourage you to speak to them to see if this program is right for you.

If you would like to use your GPMP with Vitality Club, you will need to provide your GP with the following information:
